Flexible Learning Transition Plan (FLTP) Portfolio
The FLTP Portfolio was developed within the Innovative Community Action Networks (ICAN) for the Department for Education and Child Development (DECD). The 190 page Portfolio is a resource for young people identified as at-risk of disengagement or are disengaged from school as a result
of multiple and complex barriers. The subject matter is both social and educational to reflect the young person’s life.
